Varicose veins

Varicose veins are veins that have become enlarged and twisted. The term commonly refers to the veins on the leg,
although varicose veins can occur elsewhere. Veins have pairs of leaflet valves to prevent blood from flowing backwards (retrograde flow or venous reflux). Leg muscles pump the veins to return blood to the heart (the skeletal-muscle pump), against the effects of gravity. When veins become varicose, the leaflets of the valves no longer meet properly, and the valves do not work (valvular incompetence). This allows blood to flow backwards and they enlarge even more. Varicose veins are most common in the superficial veins of the legs, which are subject to high pressure when standing. Besides being a cosmetic problem, varicose veins can be painful, especially when standing. Severe long-standing varicose veins can lead to leg swelling, venous eczema, skin thickening (lipodermatosclerosis) and ulceration. Life-threatening complications are uncommon, but varicose veins may be confused with deep vein thrombosis, which may be life-threatening.
Non-surgical treatments include sclerotherapy, elastic stockings, leg elevation and exercise. The traditional surgical treatment has been vein stripping to remove the affected veins. Newer, less invasive treatments which seal the main leaking vein are available. Alternative techniques, such as ultrasound-guided foam sclerotherapy, radiofrequency ablation and endovenous laser treatment, are available as well. Because most of the blood in the legs is returned by the deep veins, the superficial veins, which return only about 10% of the total blood of the legs, can usually be removed or ablated without serious harm.
Secondary varicose veins are those developing as collateral pathways, typically after stenosis or occlusion of the deep veins, a common sequel of extensive deep venous thrombosis (DVT). Treatment options are usually support stockings, occasionally sclerotherapy and rarely, limited surgery.
Varicose veins are distinguished from reticular veins (blue veins) and telangiectasias (spider veins), which also involve valvular insufficiency,[6] by the size and location of the veins. Many patients who suffer with varicose veins seek out the assistance of physicians who specialize in vein care or peripheral vascular disease. These physicians include vascular surgeons, phlebologists or interventional radiologists.
Signs and symptoms :-
Aching, heavy legs (often worse at night and after exercise).
Appearance of spider veins (telangiectasia) in the affected leg.
Ankle swelling, especially in evening.
A brownish-yellow shiny skin discoloration near the affected veins.
Redness, dryness, and itchiness of areas of skin, termed stasis dermatitis or venous eczema, because of waste products building up in the leg.
Cramps may develop especially when making a sudden move as standing up.
Minor injuries to the area may bleed more than normal or take a long time to heal.
In some people the skin above the ankle may shrink (lipodermatosclerosis) because the fat underneath the skin becomes hard.
Restless legs syndrome appears to be a common overlapping clinical syndrome in patients with varicose veins and other chronic venous insufficiency.
Whitened, irregular scar-like patches can appear at the ankles. This is known as atrophie blanche.
Most varicose veins are reasonably benign, but severe varicosities can lead to major complications, due to the poor circulation through the affected limb.
Pain, tenderness, heaviness, inability to walk or stand for long hours, thus hindering work
Skin conditions / Dermatitis which could predispose skin loss
Skin ulcers especially near the ankle, usually referred to as venous ulcers.
Development of carcinoma or sarcoma in longstanding venous ulcers. Over 100 reported cases of malignant transformation have been reported at a rate reported as 0.4% to 1%.
Severe bleeding from minor trauma, of particular concern in the elderly.
Blood clotting within affected veins, termed superficial thrombophlebitis. These are frequently isolated to the superficial veins, but can extend into deep veins, becoming a more serious problem.
Acute fat necrosis can occur, especially at the ankle  of overweight patients with varicose veins. Females are more frequently affected than males.
Varicose veins are more common in women than in men, and are linked with heredity.  Other related factors are pregnancy, obesity, menopause, aging, prolonged standing, leg injury and abdominal straining. Varicose veins are unlikely to be caused by crossing the legs or ankles.   Less commonly, but not exceptionally, varicose veins can be due to other causes, as post phlebitic obstruction or incontinence, venous and arteriovenous malformations
More recent research has shown the importance of pelvic vein reflux (PVR) in the development of varicose veins. Hobbs showed varicose veins in the legs could be due to ovarian vein reflux  and Lumley and his team showed recurrent varicose veins could be due to ovarian vein reflux.
 Whiteley and his team reported that both ovarian and internal iliac vein reflux causes leg varicose veins and that this condition affects 14% of women with varicose veins or 20% of women who have had vaginal delivery and have leg varicose veins.  In addition evidence suggests that failing to look for, and treat pelvic vein reflux can be a cause of recurrent varicose veins.
There is increasing evidence for the role of incompetent Perforator veins (or "perforators") in the formation of varicose veins.

    Imbalance of sugar/carbohydrates metabolism is diabetes. The food stuff is converted to essential sugar for caloric utilization of the body. Guduchi (Tinospora cordifolia) : This herb is a master-key for treating diabetes. There are many herbs to reduce blood sugar but very few contribute to improve the function of pancreas. Guduchi ranks top in this segment. It is proven remedy to treat diabetic retinopathies, neuropathies, gastropathies, nephropathies, diabetic foot injuries, diabetic complications associated with CNS, onslaught on the liver, heart, cholesterol, diabetic seminal weakness, osteoporosis etc. The clinical miraculous effect of this herb is that it can even prevent the effect of diabetes on foetal development. Insulin is secreted by ß cells in the pancreas whereas some autoimmune cells destroy them and trigger the disease. This herb has some exclusive properties to deactivate the autoimmune cells and thereby help to improve the insulin secretion.
Meshshringi (Gemnema sylvestere) : The herb is also termed as Gudmar where ‘Gud’ indicates sweet and ‘Mar’ indicates antagonist activity. Therefore the word ‘Gudmar’ indicates the activity as “antagonist of sweet”. The herb acts by different modes. 1) It regenerates the cells of islets of langerhans in the pancreas. 2) Increases the secretion of insulin. 3) Prevents the absorption of sugar from at the intestinal level. 4) The converted glucose from glycogen is utilized by the muscles and thereby helps to maintain the blood sugar level up to the desired healthy level.
Nimb Leaf (Azadirachta indica) : Neem reduces the requirement of insulin from 60 to 70%, hence it is especially effective in insulin dependent diabetes. Diabetes causes hardness in the blood vessels termed as atherosclerosis. This condition obstructs the oxygenated blood flow to the vital organs of the body which may lead to gangrene and finally amputation. The property of Neem improves the circulation and minimizes the chances of such consequence. Diabetes also complicates many skin disorders. The effect of Neem in treating skin ailments is known to every Indian.
Saptarangi (Casearia esculenta) : The herb, Casearia esculenta is especially effective in treating Insulin resistance (IR), which also aids the uptake of blood sugar by the muscular tissue and helps in lowering the blood sugar levels. The increased sugar causes onslaught of free radicals on the organs like liver, kidneys and retina. The antioxidants in this herb are capable to prevent this attack and protect these vital organs.
Amalaki (Phyllanthus emblica) : The fruit is a natural source of vitamin C which imparts supreme immunity. Regular consumption of this fruit reduces the fasting, postprandial blood sugar and also glycosylated hemoglobin. Apart from that, it has lipid lowering effect which checks the increased cholesterol and triglycerides.
Bilwa patra (Aegle marmelos) : Diabetic neuropathies cause severe pain in various areas of the body. The leaves of this herb are especially effective in treating the neuropathies and keep a check on the tenderness. Apart from this, it is also effective in preventing the diabetic nephropathies and prevent the formation of early cataract, which is a consequence of diabetes.
Haridra (Curcuma longa) : It is a common culinary item found in every house in India and recognized as Haldi or turmeric. Its use in cancer, inflammations, blood occlusion due to thrombus, atherosclerosis, neuro-protection, memory enhancement, Parkinson’s disease, osteo-arthritis, coronary artery disease, infection, ageing disorder, psoriasis and epilepsy have been strongly documented. In case of pregnancy, it is effective in proper closure of the neural tube and thereby protect the consequences like spina bifida.
Jamun seeds (Syzygium cumini) : This fruit is considered to be very useful in diabetics. Practically, the seeds of this fruit are benefitting, not the pulp or juice. The pulp and juice increase the blood sugar level and can create a serious situation for diabetics. The use of seeds in treating diabetes is well known since long time. In fact, the hypoglycemic activity of these seeds was known even before insulin was discovered. It is studied that Jamun seeds increase the insulin level in the blood by three times and also keeps check on serum cholesterol.
Karela (Momordica charantia) : This fruit stimulates the secretion of insulin from pancreas, prevents the absorption of glucose from the intestine and also improves the uptake by muscles. This 3 way action plays a useful role in the treatment of diabetes.
Kirattikta (Swertia chirata) : Whether a case of diabetes or not, dropping blood sugar level below 70 mg/dl is called hypoglycemia. Care must always be taken to prevent arriving to such situation, particularly while treating diabetes. Hypoglycemia can lead to symptoms like mental confusion, giddiness, tremors, appetite, headache, mental irritability, bradycardia, palpitation, pallor, profuse sweating, and weakness. This herb has a peculiar property that it reduces the blood sugar level but doesn’t allow to drop below normal level.
Kulinjan (Alpinia galanga) : Apart from hypoglycemic activity, this herb imparts protective effect of free radicals generated due to diabetes on the kidneys and keeps check cholesterol levels.
Trivrut (Operculina turpethum) : The complications of diabetes include burning palms and feet, indigestion, constipation and dryness of throat. This herb is effective in tackling these symptoms.
Vijaysar (Pterocarpus marsupium) : It improves the absorption of sugar from circulating blood to the muscles which lowers the blood sugar levels. This absorption improves muscular strength, overcomes fatigue, muscular weakness, cramps and restores energy level. It also enhances the insulin secretion, controls blood lipid levels and thereby manages various aspects of diabetes.
Yashtimadhu (Glycyrrhiza glabra) : Out of thousands of herbs from the nature’s treasury, very few herbs are sweet in taste, one of which is Yashtimadhu. It will be shocking to know that how sweet herb could be acting on diabetes. Yashtimadhu contains an active principle named as Amorfrutin. Current research at the Max Planck Institute for Molecular Genetics in Berlin has found that Liquorice root contains amorfrutins. Amorfrutins exhibit anti-inflammatory properties, decrease blood sugar levels and above all they accomplish this without significant adverse effects. Amofrutins were found to bind and activate a nuclear receptor protein (peroxisome proliferator-activated receptor gamma (PPARγ)) that affects glucose and lipid metabolism; important factors in type 2 Diabetes.
Each coated tablet contains powders of :
Daruharidra (Berberis aristata), Devdar (Cedrus deodara), Guduchi (Tinospora cordifolia), Jamun Seed (Eugenia jambolana), Karvellak (Momordica charantia), Nagarmotha (Cyperus rotundus), Triphala Each 75 mg; Micronized and Potentised with Guduchi (Tinospora cordifolia) Q. S.
Brief action of the ingredients in Madhumukta tablets :
Apart from Guduchi, Jamun seed and Karela, the tablets contain few additional ingredients like Daruharidra, Devdar, Nagarmotha and Triphala. The therapeutic activities of these herbs along with a nanonizing manufacturing technique are the special attributes in case of tablets. Triturating the composition with active herbal ingredient potentizes the formula and contributes to its dynamic clinical profile. The ingredients in liquid and tablets are different and their mode of action is also different. Hence they can’t be taken as a substitute for each other. The tablets contain volatile oils which could not be incorporated in liquid as they may lose the properties due to boiling. Diabetics are recommended to use both simultaneously for achieving optimum benefits.
Daruharidra (Berberis aristata) : The herb is helpful in controlling hyperglycemia and simultaneously maintains the equilibrium of serum lipids. LDL and HDL are the two types of lipids in the body. In relation to cardiac health, LDL (Low Density Lipoproteins also called as Bad Cholesterol) should be less than 100 mg/deciliter. There are many herbs effective in lowering the LDL but only few can increase HDL (High Density Lipoproteins or Good Cholesterol). The HDL should be more than 60 mg/dl and surprisingly Daruharidra (Berberis aristata) possesses this unique property. Hence this herb is a boon to diabetics to prevent circulatory complications and thereby protect the heart.
Deodar (Cedrus deodara) : Generally liver problem is always correlated to alcoholism. But diabetes also has a serious impact on liver causing Fatty liver. This herb has immense potential to repair fatty liver due to diabetes. SGOT, SGPT, Cholesterol, Triglycerides are the biochemical parameters to determine liver health and this herb brings all these parameters to normal level. Apart from this, this herb possesses supreme antioxidant properties.
Nagarmotha (Cyperus rotundus) : The mode of action of this herb is quite different. Due to Alpha-glucosidase inhibitors and Alfa Amylase inhibitors, it allows the absorption of dietary sugars in the blood very slowly. Hence, it role in post-prandial sugar control is significant. In such situation, the dietary carbohydrate gives stimulation to the insulin for longer duration. Burning palms, feet, indigestion, obesity and prevention from infection are special attributes of this herb.

Consuming any medicine daily for long time reduces the therapeutic effect as the body becomes used to and reduces the response to the medication. Keeping a weekly off, triggers the activity with improved efficacy.
Do not stop the previous medication abruptly. Keep an eye on the sugar levels regularly. If the blood sugar level drops, it may lead to hypoglycemia. Hence it is always advised to keep a pouch of glucose or a chocolate for tackling the situation. Consuming MADHUMUKTA after meals may seldom cause hyperacidity. Hence, it is recommended before meals with more dilution with cold water. Experiencing hyperacidity is indeed a sign of positive action of the formula. On this subject i mostly talked regularly to my patient..that u should eat Ghee ..mainly a Cow ghee regularly...!!
I can talked hourly on this subject...!! But seriously this is become a very important subject ,n it must should share with people about concept of Cow ghee and it's mechanism....!!
Friends..Cow ghee is Desi Ghee or clarified butter oil - often blamed for obesity and heart diseases - is not that bad after all.
Indian scientists have just discovered that cow ghee could protect us from cancer,heart disease and to improve our digestion...
Cow ghee enhances the availability of enzymes responsible for detoxification of cancer-causing substances and decreases the availability of those responsible for activation of carcinogens...(scientists from the National Dairy Research Institute (NDRI) have reported in the latest issue of the Indian Journal of Medicine)
Ghee poses no danger to cardiac health mechanism of ghee's having a protective properties.
One probable factor in cow ghee is the presence of conjugated linoleic acid (CLA), which is known to possess beneficial properties.

Preservative: Ghee contains no water, so it does not spoil easily and hence preserves the original freshness and potency of herbs and foods. Therefore, no refrigeration is required for ghee. It is ancient Ayurvedic and folk assumption that as Ghee becomes aged, though its taste becomes slightly bitter its effectiveness and healing properties increases. In many Ayurvedic classical formulation 5 to 10 years older Ghee is highly recommended. Properties of individual ingredients:
Deodar (Cedrus deodara): Lipoxygenase plays a key role in the process of inflammation and this herb has an established lipoxygenase inhibitory activity. Further, it has a pronounced pain controlling activity.
Erandmool (Ricinus communis): Ayurveda states “आमवातगजेन्द्रस्य शरीरवनचारिणः l एक एव निहन्ताऽयमैरण्डस्नेहकेसरी ll” which means, “For an intoxicated elephant called Amavata (Rheumatoid Arthritis) dwelling and destroying the garden called body at will, there is only one controller and that is a Lion named Eranda Sneha (Castor oil).”
Diclofenac Sodium is a known analgesic in modern medicine. It is effective in pain management of even unknown etiology. The comparative study of therapeutic activity WRT Erandmool and Diclofenac Sodium was performed. Erandmool was proved to be equally effective and safe.
Haadjoda (Cessus quadrangularis): Ayurveda mentions that an individual should perform exercise only up to his half capacity of his physical strength. If it exceeds the defined limit, the result is erosion and swelling of the joints. Due to ignorance, some young people perform such painstaking exercise for rapid weight loss or body building and eventually suffer from arthritis. It is researched that this herb is amazingly effective in restructuring the joint erosion in such cases.
Shallaki (Boswellia serrata): The gum of this herb is used as an active ingredient. Boswellic acid is the dynamic constituent of this gum. It is clinically proved and established that Boswellic acid helps to limit the inflammation, pain and improves mobility in very short period. It helps in both, Osteoarthritis and Rheumatoid arthritis as it inhibits Leukotrienes and Prostaglandins, the key elements responsible for inflammatory disorders.
Shuddha Guggul (Balsamodendron mukul): It’s again a gum derived from the said plant. It is not only anti-inflammatory but specially identified as anti-arthritic. It is also effective in balancing the lipid levels in the blood and an established ingredient for weight management. Its dual utility is amazing because of the weight lowering effect and anti-arthritic activity. Practically, reducing body weight also helps to lower the damage on the joints. It has proven Prostaglandin inhibitory activity.
Shuddha Kuchala (Strychnous nux-vomica): It is an excellent and rapid acting regimen for pain management, however it is poisonous too. In this sequel, it proves the dictum “Poison can be as effective as nectar if used intelligently and nectar can be as harmful as poison if used without proper knowledge”. Ayurveda has described an authentic process for purification to eliminate the toxic element and fortify the active medicinal constituent. Strychnine and Brucine are the two toxic elements in this herb. Strychnine is highly poisonous.
          The said purification process removes the Strychnine and converts Brucine into Isobrucine which is an incredible painkiller. It acts as a local analgesic and also effective in controlling the pain receptors in the brain (CNS). After appropriate purification, the hidden constructive benefits also become apparent. It helps to sharpen memory, improves humoral immunity, corrects the liver function, reduces the relapse of epileptic seizures, effective in some types of cancers, corrects digestion and also helps to purge the snake venom. It is highly recommended in Rheumatoid arthritis owing to the property to achieve stability of physiological status.

Mahamasha Tel, Narayan Tel Each 2 ml; Vishagarbha Tel, Gandhapuro Tel (Gaultheria fragrantissima) Each 3 ml

Mahamasha Tel: This is an age old formula recommended in Paralysis, Stiffness in jaws, Facial paralysis, Neck pain, Stiffness in joints, Diseases of the back bone (Spondylosis), Blunt injuries, Rheumatoid and Osteoarthritis. Its special therapeutic role in treating old-age degenerative diseases is promising.
Narayan Tel: This is traditionally used in body-ache due to long distance travelling, Osteoarthritis, Rheumatoid arthritis, Gout, diseases of the back bone (Spondylosis) and especially recommended for strengthening the skeletal bones.
Gandhapura Tel (Indian Wintergreen oil): The plant remains evergreen even in the snowfall, hence called Wintergreen. The yellowish aromatic oil derived from this plant is used worldwide. It contains Methyl salicylate, a renowned painkiller. It smells like eucalyptus oil, recommended for external use only. It imparts warmth when applied on the skin, improves subcutaneous blood circulation and helps in pain management. It should not be used on delicate skin, cracked skin, infective skin diseases and should be kept away from eye contact.

Today..Lifestyle and Ayurveda

Friends..we moving very fast..And trying to capture our target timing...
We slept less..we drink a water less..we eat vitamin less..we exercise..doing only by mouth saying..that yes I do.
But that also we do less..
That all creates..a digestion.
Early morning getting up is good ..but you should sleep early very important..
Most of the in metro starts after 10 pm.this people slept till 11 am in morning and then their food goes..,how will body this type of lifestyle...
They says..we have to work at night we get more expenses..than we have to do it..
But have a constipation problem...neck ,lumbar pain problem..please do some thing that I can go to work.....
Some are get up early in morning..come at 7 pm.,do all stuffs..And then also..slept at 12 to 1 am...less consuming sleep increase the stress and decrease your good thought process...that leads to anger.
Which helps hormones to disturb..leads Thyroid problem,menstrual problem in ladies...
Some are only on external foods...they carry there body weight...regularly..And increased.
Many times in TV advertisement...we saw..Sanitary pads..or menstrual pads advertisement that..women's feels comfortable and can go anywhere...with safety...Some adds says that She can Run,doing hard work...easily...!!
Menstrual shedding of blood...which happens every month...And 3 to 4 days..letting of blood should go proper must..if this is not going through rightly..then..periods differs with comes early or happen..or sometimes painful menstruation is happen.....
Ancient says..women should take rest during this means her menstrual will goes properly..And she will be fine...but now a days it's not possible to take every month rest for 3 to 4 days by taking leave..from job.
Or from house work....
So thoughts behind the rest is to proper menstruation...u can go to work..but to remember with not too much exercise...taking a oil free proper menstruation during this days...
Rice wash water...or a kanji is beneficial during that days to means easy flow...
Advertisement shows hard exercises..running....this all are affecting to body. as well as your menses.
Rest should be in body and mind...
Salads and high water intake..juices,coconut water...are helpful this days...
Natural irons and natural protein are always..beneficial..
Jaggery and peanuts are rich in iron and with milk early morning good for uterus....And body...
Weekly thrice Yoga exercise..withPranayam.
Anulom & Vilom.
Streching of body...Power Yoga is good for health.always...
Eat with understand with body..3 to 4 small meal in day..with lots of good..always eat with less.And eat after 4 hour..with salads..froots..buttermilk..peanuts..fried Chana...And avoid tea
Green becoming popular is already in Ayurveda..what is just a Bilwa patra+Ashwangandha+ Sunth& Pimpli + sweet herbs leafs
That are good antioxidant...that keep you energetic and sugar free...

ABSTRACT: Todays changing life style and drinking chilled bevarages etc
have became cause for predispositio of gilayu(acute tosilitis) is leading to
sore throat ,dificulty in swallowing,pyrexia and various throat infections.
A Clinical study was conducted where 60 patients selected
randomly & divide into 2 same groups.Group A was treated with trushan
churna and Group B was treated with gandhak rasayan.A clinical assessment
was done on the basis of grading criteria.trushan churna shows better effect
to reducing the symptoms of tonsilitis.
KEYWORDS:Acute tonsilitis,Trushan Churna,Gandhak Rasayan etc.
INTRODUCTION - Palatine tonsils are a part of lymphatic system,are
lymph nodes and plays an important role in immune defence system.there is
antigen-antibody reaction to fight fron infection & illness.antibiotics may
supress the infection temporarly but there is chances of recurrence.
In Ayuveda.Gilayu is somewhat related to acute tosilitis and
dominant doshas are kapha & rakta .As per ayurvedic classics,Trushan
churna has ushna,tikshna with raktshodhak & kaphaghn,can be
used in Gilayu.
As per ayurveda,ayurvedic medicines acts from the root of
the disease with less or without side effects and improves immune
system.Trushan churna is katu,tikshna,ushna,raktashodhak,kaphaghn used
in management of gilayu.

INCLUSION CRITERIA-pt.having signs& symptoms of acute tosilitis.
-Age between 10-60 yrs
-Written consent taken from the pateint.
EXCLUSION CRITERIA- Pt, with major systemic disorders
e.g.DM,HTN.IHD etc.
-Vincet angina
-Necrotic tonsils
-Tosillar foreign body

DISCUSSION-Tonsils are known as “guards or oral cavity”,is a part of
immune system of the body.they fight from infections & illness.inflammation
of tonsils called tonsillitis.due to micro organism,s infection,due to exposure
to environmental factors,change of climate causes infections.mostly school
going childrens are effected due to low immunity.tonsilitis can be correlated
with gilayu as described by sushrut.
According to Ayurveda,Gilayu is kaphaj and raktaj dominant entity.sushrut
advised surgical treatment for Gilayu wherever Vagbhata & Chakradatta
advised katu,tikta,ushna dravyas(medical management).in modern
science,conservative treatment like antibiotics,analgesics,anti-inflammatory
drugs are available,but they all having limitations and side effects i.e.allergic
reactions,contra-indication in children,pregnant and lactacting women.long
term use may cause harmful results and its also vey costly,cant be afforded
by low socio-economic,due to this,it is neccessory to use such kind
of treatment which has less side effect and also cost effective.
In gilayu,we need kaphagn and raktshodhak chikitsa.trushan churns has
katu rasa,ushna veerya.tikshna,ruksha,sookshma guna.hence,works as
kaphashamak &raktshodhak.honey was used as anupan as it is yogvahi.
Gandhak rasayan has already proved its anti microbial and antiinflammatory
result hence it is ed in control group.
By statistical analysis,it was found that in all parameters paired t test is
significant in both group but when we compare both groups by applying
unpaired t test,the

CONCLUSION-After statistical analysis,it is conducted that oral trushan
churna is effective than control group in Gilayu.
It is safe,cost effective,easily available.
It decreases the severity of throat infection.
Further research with new ideas can be done-
By increasing the samole siz.
By making in ointment form which can be applied locally.

Polycystic ovarian syndrome...or we a can say..PCOD..Polycystic ovarian disease
Actually Modern the disease and then followed by treatment are going on...
But I eagarly wants to share one thing to with my ayurvedic fellow doctors..that..we are practising..ayurveda...And we must take help..of Modern diagnostic test..for just our knowledge..but not take treatment by that manner....!!
Our Pathy and our principles of treatment is absolutely different from modern treatment...
We based on Tri dosha ,Sapt Dhatu,and Tri mala..And 13 strotasa...we should look for this fundamental classical Ayurveda...!!
Most common occur.. menses..or delay menses...
Inability to conceive
In ability to have baby..
Then goes stress..
And improper timetable of diet..
Junk food...stress work..long hour duties..
Habbit of taking hormonal tablets..
Result less work...less exercise..
And weight gain..
Final something this like disease..
What exactly happen...Here we see starts with indigestion..where Pitta dosha is responsible for proper digestion..And patient ignore..their body signals...constipation..still patient ignore and go on....
Then gases formation in abdomen which we may called a pakwashayagat vat...PWGV..
U said PCOS..we also make PWGV...jokes apart...
This vat tries to comes out...but can't..Then it find empty cavities in body...we can say..Garbhashay,Mutrashay,or in Pakwashay...any will go there and sense it's presence by giving disturb..pitta making bad digetion and kapha dosha..which present in disturbs it also.we called kled..or mucus membrane or endothelial membrane of utreus..that leads to increase in size endometruim...which goes 7..8.10..13... mm
And Menses.or comes with delay...also..
This kapha doaha which produce in digestion are disturb...And that leads to increase in kled..which mixes with blood...And slowly it disturbs the fatty acid..And cholesterol...Mechanism..
Which leads to agnimandya...And gaining of weight..Stholya..
This kled or kapha with the help of vat..Vyan goes to ashay...if it pakwashay..Then it leads to Grahani..or we co relate with IBS..
If it goes to increases the creatinine or uric acid...or formation Stone..
And if it surrounded at around the called cyst..with sticky...bulky..fluid like kled.
That's kled...kapha dosha are responsible...for formation Cyst..which are responsible to make difficulties in ruputurs of ovaries..on perticuler day...if it not rupture on time then again delay menses...And inability to conceive....!!
So after this samprapti...SMP...WE CAN SAY..
that VatKapha dosha..are responsible for cyst formation...
Or Vat Pitta Dosha are responsible for cyst to cut this..
We go to basic......
Diet...on time...proper...
And hectic free life living...after is more important than any other else.....
Then treatment on proper digetion...
To regulate Pitta dosha we called Pachak means proper kled will produce...
And what about that already residential...happy family excessive kapaha....which present around and inside the body...after regulating pitta dosha...we have think kapha and vat...I
Saman vayu and vyan vayu...who take kleda to all over body in proper manner..but here we have to cure Vat dosha also..with kapha dosha....
So treatment after pachana..on vatkapha dosha to minimise the excessive vat and kapha dosha is shaman and shodhan chikitsa....
We can starts with wonderful medicine which given in astang hruday...on vatkpahshamak....
By giving just for 2 months Cyst will minimises...And reguler rupture of ovaries..get started....
And reguler Menses also comes....
Here with the help of Panchakarma..we can remove this excessive...kkapha and vat dosha...also by Vaman

A preparatory groundwork for conception.....
Panchakarma is a foremost set of treatment modalities in Ayurveda, which includes Vaman (induced emesis), Virechan (induced Purgation with oral medicines), Basti (Rectal administration of various formulations in the form of enema), Nasya (Administration of medicines through nasal route), Raktomokshan (Removing specific quantity of blood from the body).
These treatments are also termed as “Shuddhi-kriya” which means these treatments eliminate the toxic metabolites from the body. In short, Panchakarma helps to clean and detoxify the functional systems of the body.
This empowers immunity to such a level that pregnant woman will hardly become victim of any disease. It is indeed very essential as there are very limited medicinal preparations considered safe in pregnancy.
Secondly, sperm is like a guest in female body. As we beautify the house for any guest, decorate flowerpots, change the curtains and bed sheets, make the house neat and tidy, same thing should be followed in welcoming this guest. The meeting of sperm with oocyte (formation of zygote) becomes exceptionally pleasant when the guest is also well dressed, polite, joyful in nature and free from any disease.
In spite of experiencing regular bowel, urine etc., is it still necessary to clean the system before planning for conception?
The answer is 'YES'. We clean and mop the house daily. When a special cleaning campaign during festival or any special occasion is undertaken, we notice a huge amount of dirt accumulated behind the furniture, odd places and corners of the house. If this special cleaning is not done periodically, the entire atmosphere remains dull and the long-term ill effects are experienced thereafter. Thus, the importance of Panchakarma is unquestionable with respect to pregnancy.

Indomethacin etc
Disadvantage: Risk of gastritis, internal bleeding, nephrotic syndrome, glmerulo nephritis by short term or long term use.
Cox-2 inhibitors
Celecoxib: action similer to NSAID’s but with less adverse affects.
Disadvantage: Dermatitis, stomatitis, protenuria, cytopenia. Exacerbation after due course.
Comparitively safe but action seen is not the same in all, very slow, majority of the patients not benefited.
Methotrexate: A well known, said to be the most effective immune modifying agent , widely used in present practice.
Disadvantage: MTX has toxic effects on hematologic, renal, GI, pulmonary, and neurologic systems. So should be used with caution. May become fatal if co administered with NSAID’s
Sulfasalazine: Another immune modifying agent usually co administered with methotrexate.
Disadvantage: weak action, but toxic.
TNF alfa inhibitors:
Eg: Infliximab, Etanercept
T-cell inhibitor
Disadvantage: Although these medicines improves the clinical symptoms, and stops the progression, the major disadvantage is serious infection and malignancies.
Disadvantage: Reduces the symptoms, but not the damage and progression.Risk of serious side affects makes it very difficult to administer for long term and also this will not check the progression.

All the medicines mentioned do not give definite improvement. Exacerbation of the symptoms during the course of treatment is very common, and the same medicine may not work further.
Adverse affects of DMRD drugs like Methotrexate which is hepatotoxic, neurotoxic and nephrotoxic many times forced to stop medication du to adverse reactioins.
Immuno modulators like TNF alfa inhibitors, T cell inhibitors etc almost always ends with severe infection, and sometimes malignancy.


n      Role of genetic factors in the regulation of RF isotype level production
n      HLA-DR3 & DR4 antigen was associated with low IgA RF
n      HLA-DR1 & DR2 antigen was associated with high IgA RF, IgM RF, AND IgE RF
n      IgM RF was positive in almost all sero positive patients and most of seronegative.
n      IgA RF was negative in sero negative patients but was mostly positive in other groups .
n      Patients with negative IgM and IgA RF had incactive or recent RA.
n      Anti nucleic antibodies with the highest values were significantly associated with high levels of IgM and IgA RF.
n      Almost all patents with extra articular features had higher level positive IgA, IgM, IgE RF when compared with RA without extra articular symptoms
n      Most of the researchers believe that IgA RF is associated with RA with poor prognosis and systemic symptoms.
n      IgE RF is associated with poor prognosis but without extra articular involvement.

We have filtered these to 16 varieties to consider the treatment. Right now, no facility to do Ig RF tests in near by place.

n      VATHARAKTHA & AMAVATHA are the two diseases where we see many similarities with the symptomatology.
n      Amavatha is a disease mentioned only in laghu thrayees that is after the 7th century, not mentioned in the previous texts.
n      Vatha raktha is the only disease mentioned in Brihath thrayees that can be co related with RA.

n      It seems that explanation regarding vatharaktha in Charaka and Sushrutha school of thought is different.
n      Lot of difference is there in the symptomatology.
n      Charaka mentions a joint condition whereas Sushrutha explains Vatharaktha briefly, but he has not given importance to the involvement of multiple joints nowhere mentioned the involvement of joint, but explains symptoms related with skin lesion, which is painful, reddish, with swelling and burning sensation or whitish with itching. But the main site of lesion is mainly feet, but sometimes hands, and symptom may spread to whole body as poisonous rat bite.
n      While explaining the symptoms he says that there may be severe tenderness at the site of lesion or sometimes numbness.
n      Sushrutha while explaining incurable vathashonitha, says that, if the lesion spreads up to knee (from foot), and ulcerates with secretion of serum, blood and pus & in case of muscle wasting & in a debilitated person,it is incurable. So here he clearly mentions ulceration in the leg, from foot up to knee as a complication and also incurable.
GAMBHIRA and UTHANA Vatharaktha are not two types according to Sushrutha
n      Sushrutha says that gambhira and Uthana are not two types of vatharaktha, but two stages of disease. This may be because Sushrutha was explaining skin condition with regards to ulcer and its stage as superficial and deep. Charakas two types depending upon the severity of illness as seen in RA.
n      So Sushruthas explanation may not be related with RA, or he may not have given much importance to vathashonitha as his main subject was surgery.
n      Hence explanation of Sushrutha holds good for Gout, Pseudogout, Peripheral vascular obstruction,ischemic ulcers,  venous ulcers etc.
n      Hence here we consider Vatharaktha of charaka as a co relative study to RA and also AMAVATHA.


n      Leading symptoms seen in sandhis, mainly phalangeal joints and toes, and then multiple joints.
n      Multiple joint pain
n      Pricking pain
n      Tenderness
n      Reccurent exacerbation and remission
n      Malaise
n      Formation of nodules
n      Skin rashes, excessive or no sweating, tingling and itching sensation over joints.
n      Joint deformities, stiffness, rigidity
n      In gambhira vatharaktha, pain is severe and paka usually occurs.
n      Almost all the features of RA is explained except morning stiffness and symmetrical involvement, which is also a diagnostic criteria to consider RA.
n      Extra articular symptoms like necrotic ulcers(paka) as seen in vasculitis, mandalothpatti (butterfly rash in sle), visarpa (skin lesions with bullous ulcers, necrotic or gangrenous ulcers with systemic symptoms)
n      So considering all the criterias, RA comes under vatharaktha of Charaka and all differential diagnosis of RA should also be considered under vatharaktha.

n      Bodyache, anorexia, malaise, heavyness, fever, lightness of the body are the common features.
n      Paka is not the symptom of amavatha (apakaha)
n      Multiple joint pain
n      Excessive urination
n      Abd distension etc are the other features.

Amavatha is mild and mild to moderate RA (and also fibromyalgia)
n      In amavatha extra articular symptoms with systemic involvemnt is not explained as in vatharaktha
n      But grahani (?enteric arthritis), abdominal pain, vomitting is said as upadrava
n      Amavatha with involvement of almost all the joints, swelling, and tridosha are considered as difficult to cure. With single dosha as sadhya, and two dosha as yapya.
n      So there is no asadhya amavatha explained in the text as in vatharaktha, vatharaktha with tridosha involvement and upadravas are asadhya.

RA AND Ukkinadkas Ayurveda
n      In our hospital we have treated more than 2800 cases of RA since last 13 years, and started keen observation since 8 years. But detailed documentation was not done. 90% of the observation is in OP level, and few of the patients might have belonged to Ankylosing spondylitis and such other collagen disorders.
For the better management we have classified RA under 3 catagories.
n      MILD (laghu) RA
n      MODERATE (madhyama) RA
n      SEVERE (theevra)RA

n      The symptoms are similar as explained before, but the deformity of joints will not be seen even after a very lengthy course of the disease. And response to the Ayurvedic treatment is very fast, like 1 to 3 weeks. The symptoms will be reduced to ¾ th with in 1 or 3 weeks.  (? NEGATIVE IgA & IgM RF TYPE OF RA)

n      In this condition also the symptoms are similar as explained earlier, but the severity of pain & other symptoms will be severe. In this case also there will not be any deformities even after a very lengthy course, but the response to the Ayurvedic treatment is slow when compared with mild RA. (? Ig E RF TYPE OF RA)
n      In this condition the signs & symptoms are very severe & most of the times deformity is seen within 6 months of onset of symptoms & the sufferings (pain & systemic manifestations) are more in this variety of RA. The response to Ayurvedic treatment is poor & slow. (? IgA RF TYPE OF RA)

n      In our Hospital, mild & moderate RA cases are treated as out patients.
    In mild & moderate cases we prescribe-Kashaya powder or Liquid kashayas, Herbal & Herbo-mineral tablets internally, and in moderate cases we add lehyas and herbal powder according to the need. For external application we use Lepa powders or oils  according to the need.         
n      In these types of patients we hospitalise the patient for three weeks in our hospital for the panchakarma treatments, which gives very good result particular group of the severe and severe to moderate cases of RA.
n      Internally we prescribe-Kashaya powder or concentrated Liquid kashayas, Herbal & Herbo-mineral tablets, lehyas and herbal powder according to the need. For external application we use Lepa powders or oils according to the need.
n      60% of RA patients visiting our hospital comes under this group
n      Shamanoushadhis & patient education
n      Short course, multi drug combination is preferred, low dosage is usually sufficient.
n      Result is seen within 3 weeks in almost all cases (90-100%).
n      Course of the treatment usually varies from 3 months up to 1 year.

MODERATE RA           
n      25% of the patients visiting our hospital comes under this group
n      Shamanoushadhi, preferably combinations with potent medicines like rasna panchaka , in higher dose, but not multi drug combinations like maharasnadi.
n      In moderate to severe cases panchakarma is preferred.
n      Good result was seen within 1.5 to 4 months in 60-80% of the patients.
n      Course of the treatment usually varies from 6 months up to 2 years or more.

15% of total RA cases visiting our hospital come under this group.
n      Very difficult to treat.
n      As deformities occur very fast, risk of systemic involvement, organ damage, and delayed action makes the treatment much difficult.
n      But still with the shamanoushadhis we get very good result in about 15 to 20 % of severe RA and moderate result in other 20% of cases. But rest of the severe RA patients develop joint deformities very fast and in such patients we have tried with the combination of shamanoushadhis and panchakarma, the response was very good and many patients who had no response earlier started improving, so now a days in severe RA we start with panchakarma treatment and the overall result raised up to 40%.
n      In rest of the severe RA patients prognosis was poor with Ayurvedic medicines alone. But integrated approach was beneficial in about another 30% of the patient
n      Course of the treatment is 2 to 5 years, may extend up to life time also in this group.

n      Since last 2 years in severe RA trying with integrated approach with DMRD(under rheumatologists prescription), presently we have such many patients under treatment, symptomatic improvement is excellent, but complete withdrawal of modern medicine is not possible in most of the cases because of fear of aggravation. But we noted remarkable improvement with the symptoms, HB and ESR in all cases after starting Ayurvedic medicines.
n      In a female patient aged 28 years, we have successfully reduced the dose of DMRD drugs to 100% with very good result. She was taking TNF alfa inhibitor and suffered from TB. So switched over to other mild DMRD, her symptoms aggravated, then she approached us, treated initially with panchakarma followed by shamanaushadhis for next two years. Now she is working in London without any symptoms and medicines. Another important thing is she had deformity in left elbow, the progression could be stopped and she is devoid of any symptoms of RA now except for the damage which already happened

n      In many cases of severe RA with unsatisfactory result, we have tried our medicines along with NSAID’s (Indomethacin 25 mg, 1 OD or BD),.
n      We have observed positive result in many cases after a very long course of treatment, in few cases we could stop the NSAID after long time.
n      Some patients had intermittent exacerbation and required NSAID’s in between.

As per our observation over all –by Ayurvedic management we can control 60% of RA cases very easily with simple treatments, other 25% of patients with some long duration. But another 15% of the patients are the most unfortunate patients with greater difficulties & still we can control the disease in 40% of the severe RA cases.
None of the system gives or claims cure or standardised treatment for RA.
Ayurvedic treatment many times proved to be a superior treatment. But evidence based study in terms of both Ayurveda and modern parameters in a vast group of patient are necessary to assess the effectiveness and safety of the treatment.    

n      In modern medicine there is no remedy for mild, mild to moderate and moderate types of RA. Other than NSAID’s, no other medicines are advisable in this stage, none of the medicines reverse the process of auto immunity, but just blocks the chain of auto immunity as well as normal immunity, so it’s systemic adverse affect restricts the use at this stage.(Now a days Rheumatologists suggest use of immunosuppressive from beginning!!)
n      If not treated some mild to moderate & moderate RA may turn in to moderate or moderate to severe RA in due course.
n      So Ayurveda is a treatment of choice for those with mild and mild to moderate and moderate RA.
n      In moderate to severe and severe cases, the main disadvantage of ayurveda.treatment is lack of effective pain killer, controlling  the pain is very important to improve the patients work efficiency.
n      In severe RA- controlling the fast progressive nature and early deformity as early as possible is a challenge in getting good result.
n      Getting result in 30 to 40% of severe RA is really not bad.
n      But in many patients the trial & error method of drug selection & delayed action makes it difficult to treat severe RA cases as the deformity is fast.
n       So by considering the various gene isotopes and other factors, we can try to make the 40% success rate of severe RA to even more. By administering the medicine in a vast group of RA patients, we can study the action of different groups of medicine in patients with different groups of gene isotopes and other factors.
n      Hopefully this may give us some guide to use specific group of medicine in particular type of RA to achieve fast and accurate result in large number of RA patients.
